SPRINGFIELD—State Representative Tom Cross (R-Oswego) encourages Illinois residents to sign up today to request Diabetes Awareness License Plates!

“It has been a goal of mine to make these plates a permanent fixture in Illinois,” said Cross. “These plates not only provide funding for Diabetes Research, but are also small traveling billboards for Diabetes Awareness that will be seen across the state and the country.”

In order for the Diabetes License Plates to be put into production, 1,500 Illinoisans must request the plates. The goal is to hit that threshold by Diabetes Awareness Day, November 14th.
“We want to have these plates available as quickly as possible and my goal is to announce we have reached the 1,500 plate minimum by November 14th, Diabetes Awareness Day,” said Rep. Mike Tryon (R-Crystal Lake).

Cross has created a web site to make the request process easier. The form to request the plates and instructions are available at

“The process for requesting the plates is very simple.  You just need to fill out the request form and send in a check for $25 to the Secretary of State’s office,” said Rep. Jim Durkin (R-Western Springs).

The form is available on Cross’ web site. The $25 will be placed in the Diabetes Research Check Off Fund. If the 1,500 plate requests are not reached, the $25 will still go to Diabetes research.

Currently only 3 other states have Diabetes Awareness plates.
